Patients have more control over end of life with new document

Minnesota’s largest healthcare organizations are working toward adopting a new medical document that allows patients to tell their family and physicians exactly what care they want at the end of their lives.

Hospitals, nursing homes, and hospices across the country are beginning to adopt Provider Orders for Life Sustaining Treatment (POLST) programs to give patients more control over how and where they die.

The POLST Paradigm Initiative Web site reports that California, Oregon, Hawaii, Washington, New York, Tennessee, West Virginia, North Carolina, and parts of Wisconsin have state programs endorsed by National POLST Paradigm Initiative Task Force.
