Malnutrition is a prevalent, but often underdiagnosed or misdiagnosed medical condition that adversely affects patient outcomes. Coders, CDI specialists, and clinicians need to understand the evolving definitions of adult and pediatric malnutrition as part of good patient care and to prevent errors of commission and omission in its diagnosis, documentation, and coding in ICD-9-CM and ICD-10-CM. Many EHRs are not well designed to support clinicians, including physicians and clinical dietitians, in assessing adult and pediatric malnutrition during a patient encounter.
Recognizing this challenge, one hospital, opted to amend its nutritional EHR assessment template and workflow to better capture clinical metrics of adult and pediatric malnutrition on the front end and communicate it with the medical staff.
In concert with coding, CDI, medical informatics, and the medical staff, the speakers supported the hospital as they incorporated ASPEN and the Academy of Nutrition and Dietetics’ malnutrition criteria in the EHR and revised their medical staff’s policy to allow dietitians to add clinically supported malnutrition to a patient's problem list. You will hear the clinician discuss the evolving definitions of adult and pediatric malnutrition and critical ICD-9-CM and ICD-10 conventions in coding these conditions when documented. You’ll also learn about the hurdles the staff faced in advocating the solution with executive leadership and the improvements they’ve seen so far.
