Pay-per-view: Getting caught in the perfect storm

Little did I know while enjoying the Fourth of July from a pontoon boat on Green Lake in Minnesota for the first time in years—since CMS did not release the OPPS proposed rule as it usually does on the eve of the Fourth, allowing me to enjoy the holiday like a normal person—that the perfect storm was already brewing.
When the proposed rule was released shortly after the Fourth, I was so pleased to see it at a relatively brief 718 pages. This length was unprecedented, and I thought it was great CMS was going to give providers a break from massive changes. But then I started reading and did a double take. Then a triple take! The 2014 OPPS Proposed Rule included the most sweeping changes since the inception of OPPS.
