Tip: Follow guidelines for reporting Interactive complexity

When specific communication factors are present that complicate the delivery of a psychiatric procedure, coders can report new add-on code 90785 for interactive complexity.
Typically a third party will be involved when this code is warranted.
Code 90785 should be reported with the appropriate code for:
- Diagnostic psychiatric evaluation (90791-90792)
- Psychotherapy (90832, 90834, 90837)
- Psychotherapy when performed with an E/M service (90833, 90836, 90838, 99201-99225, 99304-99337, 99341-99350)
- Group psychotherapy (90853)
The add-on code does not include a time element and should only be reported once per session. Time spent by the provider will be reported in the main service code.
Do not report 90785 with a regular E/M code. It is only for use when the patient receives psychotherapy.
