ACDIS members have access to the Forms & Tools Library which includes:
- Query forms

Recent updates include:
- Six different policies and procedures donated by Donald A. Butler, RN, BSN, Manager of Clinical Documentation at Vidant Medical Center in Greenville, NC
- A 26-card documentation pocket guide from Robert S. Hodges, BSN, MSN, RN, clinical documentation improvement specialist at Aleda E. Lutz VAMC in Saginaw, MI
- A CDI program survey form for administration and physicians developed by James S. Kennedy, MD, CCS, of FTI Consulting and Ron Tapnio of Presence Health
- A PowerPoint presentation on pediatric malnutrition, including definitions, classifications, and an assessment scoring sheet provided by Robert S. Gold, MD, of Atlanta

Please note that the slides for the general session (“Medicare Claims Recoupment: Case Study of Department of Justice Settlement”) were recently re-uploaded. ACDIS wishes to notify attendees at the 5th Annual ACDIS Conference on May 10, 2012 that these slides have been corrected to refer to clinical documentation improvement (CDI) programs generally. CDMP® is not a generic term, and any reference to CDMP® during the presentation was not intended to refer to the proprietary CDI methodology provided through J.A. Thomas and Associates (JATA) or in use at any of its hospital clients. JATA owns U.S. trademark registrations for CDMP, CDMP GUIDE and CDMP QUALITY, COMPLIANT DOCUMENTATION MANAGEMENT PROGRAM and CLINICAL DOCUMENTATION MANAGEMENT PROGRAM.
A volunteer committee will begin auditing the ACDIS Forms & Tools Library later this month. Please take a moment to complete this short survey regarding the ACDIS website.
