A number of diagnoses in ICD-9-CM require more documentation from physicians to be accurately assigned in ICD-10-CM. Many of these diagnoses are commonly seen in hospital outpatient medical records.

Coders can help clinicians start preparing for the October 1, 2013 implementation of ICD-10-CM by pointing out areas where ICD-10-CM requires additional documentation.
