Looking for a starting point in your quest to clean up modifier usage at your organization? Begin by reviewing the following modifiers, which often cause trouble for coders.

Modifier -25

One commonly misused modifier is -25. You should use this modifier only to identify an E/M service that is significant and separately identifiable from a procedure that was performed on the same date of service.
