Tip: Detailed requirements govern trauma activation reporting

Field activation trauma is charged with the 068x revenue code series based on a hospital’s trauma level certification. ED visits levels are charged based on its ED visit level guidelines. Critical care is charged if criteria for CPT code 99291 are met.

If both 068x and criteria for 99291 are met, report HCPCS code G0390 with the 068x charge.

If field activation is not met, a hospital may charge trauma activation fees, but must do so with revenue code 0450. This sums with the ED visit level charges or the critical care charge with a unit of one.

It is appropriate for hospitals to report trauma activation charges for in hospital/ED activations when the same costs are incurred, but pre-hospital notification criteria for revenue code 068x is not met.
