Hybrid is a healthy state for vegetables, but not for an EHR

Throughout the past five years, I have written many articles on topics related to the transitioning role of the HIM manager and the legal health record as automation continues to advance in healthcare facilities. During that time, technology has improved the interoperability of systems, legislation has encouraged the implementation of EHRs, and healthcare providers have begun recognizing the importance of EDMS, work flow solutions, and business intelligence as critical components of any EHR strategy.
 
At times, I’m encouraged by the increased adoption of software, but other times, I am still discouraged with the seemingly slow progress in some areas. One example where the health informatics field can use some shaking up is in its casual promotion of hybrid records.
 
Despite common knowledge that hybrid systems are worse than either fully paper or electronic records, we continue to see proliferation of models implemented that promote the hybrid record as an extended transition strategy. However, with proper planning, you can avoid entering into a hybrid state.
