Review tags programs that help complex patients

Patients with chronic illnesses and complex health needs present many challenges to hospitals. A new review has shed some light on some of the best types of programs to help them.
Successful models, according to researchers at Johns Hopkins University, included strategies to identify patients who were most likely to benefit from an intervention, the use of evidence-based planning and monitoring strategies, engaging the family and patient in care, and improving transitions from the hospital to other levels of care.
