Reducing readmissions: Study reveals which strategies work best

What strategies are effective in reducing readmissions? That was the question a team of researchers at the Yale School of Public Health sought to answer through a new study backed by The Commonwealth Fund, a nonprofit devoted to improving healthcare quality.
Researchers asked nearly 500 hospitals participating in two separate readmission-prevention initiatives what strategies they were using to prevent patients from needlessly coming back to the hospital—and whether their efforts were succeeding in reducing readmission rates. They found that making sure patients had a follow-up appointment with their doctor after discharge was the only effort that consistently reduced readmissions. They also found that using three or more readmission-reduction strategies helped to make a difference as well.
