Intervention reduces same-cause readmissions by 70%

A hospital transition program implemented by a Massachusetts health maintenance organization (HMO) successfully boosted medication adherence and reduced readmissions by over the course of a year.
Catamaran’s Hospital Transition Program, was a partnership between Catamaran Corp., a provider of pharmacy benefit management services and technology, and Health New England, Inc., a Massachusetts HMO. The program targeted patients taking more than six chronic medications at once. The program sought to improve data sharing and communication and provided monitoring, education, and intervention for patients once they were discharged from the hospital. The result: A 70% drop in same-cause readmissions and a 50% drop in all cause readmissions, when compared to a control group.
