Study shows coordinated care works to prevent readmissions

Using a team approach to support the transition of elderly patients from the hospital to the next level of care can help prevent readmissions, according to a study in The American Journal of Managed Care.

The study focused in patients 60 and older who left an academic medical center for another level of care. Some patients were cared for by using a primary-care based transitional care group, which consisted of a team of practitioners including doctors, pharmacists, and social workers, whereas others received standard care.
Patients who were cared for by the team saw a reduction in readmissions of between 11.7% and 17.3% in some cases.
