Beefing up discharge summaries benefits heart patients

Want to help your heart patients transition home successfully? Provide their primary care physicians with a better discharge summary, says a study published in Circulation: Cardiovascular Quality and Outcomes.

A team led by Dr. Leora Horwitz, adjunct associate professor of internal medicine at Yale School of Medicine and director of the Center for Healthcare Innovation and Delivery Science at New York University Langone Medical Center, analyzed more than 1,500 discharge summaries from 46 hospitals across the country. The team found that none of the hospitals consistently produced summaries that contained all the needed information. With this in mind, it might be time to perform a review of the summaries at your own facility.
