MDS 2.0 tip of the week: The RAP summary


Based on review of the assessment information and the resulting conclusions drawn from the analysis, the next step is for the interdisciplinary team to decide whether or not the triggered condition affects the resident's functional status or well-being and whether it warrants a care plan intervention. This information is documented in the RAP summary. Also known as Section V of the MDS, the RAP summary collects information about which RAPs are triggered and which RAPs are care planned.
