Free form: Letter denying application request
Credentialing & Verification Update, January 12, 2009
On certain Fridays, the Credentialing Resource Center (CRC) Blog publishes a free credentialing form. Last week Associate Editor Emily Berry posted a sample letter denying a request to process an application for medical staff membership. The form can be customized to deny a request for privileges, or it can be edited to include your organization’s criteria.

If there is a particular form you would like to see
included on the CRC Blog, or if you have a form you would like to share with fellow MSPs, please email Emily at eberry@hcpro.com.
