ACDIS members have access to a collection of sample Forms & Tools donated by their peers. Recent additions include:

  • Sample queries regarding HIV status, abnormal biliary radiology findings, general radiology findings, pathology findings, and bowel function donated by Paul Evans, RHIA, CCS, CCS-P, supervisor of clinical documentation integrity at Sutter Health in San Francisco.
  • Sample physician queries regarding functional quadriplegia, non-treating provider (radiologist/pathologist) findings, and Ileus, donated by Katy Good, RN, of Flagstaff (AZ) Medical Center.
  • A sample query regarding excisional debridement donated by Vicki Davis, RN, CDI specialist at Alamance Regional Medical Center, Burlington, NC.
ACDIS is currently recruiting volunteers to help review materials located in the Forms & Tools Library as we begin efforts to upgrade the ACDIS website. Volunteers must be familiar with the Forms & Tools Library as well as with AHIMA and other industry guidance regarding physician queries and related industry best practices. ACDIS members and CDI Strategies subscribers will receive a brief survey regarding ACDIS website improvements in the coming week. We encourage you to participate.
