ACDIS Annual Awards Nomination Form

Nominees sought for 2010 CDI Professional of the Year

The nomination period for the 2010 CDI Professional of the Year award is open through March 15, 2010. The 2010 ACDIS annual conference committee will choose one CDI Professional of the Year Award winner and two winners of Recognition of CDI Professional Achievement.

The winner of the Professional of the Year award will receive one free admission to the 2010 ACDIS third annual conference in Chicago. All three winners will be recognized before their peers at the conference’s networking and awards luncheon on Thursday, June 3.

Nominees should be someone who has a passion for the CDI profession and adheres to the ACDIS Code of Ethics. Consider nominating those who may have made a difference to your hospital’s CDI program or someone who positively influenced your CDI team.

To submit a nomination, download the attached CDI Professional of the Year nomination form. After you fill out the form, submit it to ACDIS Director Brian Murphy via e-mail at bmurphy@cdiassociation.com.
