A mechanical error appears to be behind a release of toxic fumes that forced a medical facility to evacuate on July 20.

A relative of a patient at the Wabash Valley Surgery Center in Terre Haute, IN, experienced breathing difficulties and was taken to a hospital for treatment, according to the Tribune-Star. Staff members at the surgery center evacuated patients successfully.

A regulating device released too much of a toxic chemical known as Tri-Act 2813 into the boiler system, causing fumes to waft out, a fire department spokesperson told the newspaper. The chemical is used to clean out boiler systems.
