Immunization group offers flu shot declination form

Obtaining documentation from healthcare workers who decline influenza vaccinations is part of the recent influenza vaccination recommendations from the Centers for Disease Control and Prevention and the Joint Commission on Accreditation of Healthcare Organizations. To help in that effort, the Immunization Action Coalition has developed "Declination of influenza vaccination."
Free use of the form is for healthcare employers who recommend that their employees receive annual influenza vaccination. The document provides reasons for vaccinating healthcare workers against influenza and space for the employee's signature if the employee declines the vaccination offer.
