Demonstrating to the JCAHO that you track and analyze performance trends, and therefore meet the standard PI 2.20, will rely a great deal on your ability to effectively document incidents. These documents must include accurate and detailed information about any incidents so you can later analyze the data and determine the causes for the recurrence of such events.
For example, on a postoperative infection report, make sure you include a space for the person completing the report to give detailed information about essential surgical components such as
- performing surgeon and staff
- pertinent dates (e.g., date of infection, date reported, and date postop completed)
- patient risk factors
- antibiotics
- dressing
- sterilizers
- treatment
This tip was adapted from Briefings on Ambulatory Accreditation, a monthly publication that reports on the activities of ambulatory care accreditors the JCAHO and the AAAHC. Click here for more information or to subscribe.
