Investigate the root cause of a medication error

The investigation may be the most important phase of handling a medication error because it can help determine what caused the error. This should include a combination of reviewing charts and interviewing the employees involved to determine how to prevent a similar error from occuring in the future.

The investigation should answer the following questions:

  • What caused the error?
  • Could it have been prevented?
  • Who was involved?
  • What can be done to prevent it from happening again?
