Society offers health history checklist

The American Society of Anesthesiologists (ASA) is offering a medical history checklist patients can fill out and carry with them in case surgery is ever needed.

The checklist, called "Anesthesia & Me©," gives patients the opportunity to identify if they have a number of medical conditions, including allergies, stomach problems, and heart disease. The ASA suggests patients fill out this checklist, date it, and carry it with them as a way to document vital information a physician may want to know if surgery, planned or unplanned, is ever needed.

"There are many ways in which a well-designed anesthesia plan contributes to the best possible satisfaction and outcome for the patient," said ASA President Orin F. Guidry, MD, on the ASA Web site. "There are questions you should ask and, just as important, there is information you should share with your anesthesiologist to help him or her design the best anesthesia plan for you."
