JCAHO announces medication reconciliation Sentinel Event

The JCAHO released a new Sentinel Event Alert last week, citing continued problems with medication reconciliation. Reconciliation is a current National Patient Safety Goal.

According to a January 25 press release from the commission, the alert was issued because reconciliation errors are still a problem, even after the goal went into effect on January 1. The alert outlines the problem and how facilities can work to properly reconcile a patient's medications throughout the continuum of care.
