In the healthcare world, the unveiling of Watergate's "Deep Throat" was quickly overshadowed by the revelation of the 2006 National Patient Safety Goals.

For safety professionals, though, the Joint Commission on Accreditation of Healthcare Organizations' list was more notable by what was missing: a mandate to reduce the risk of surgical fires.
