The state of Wisconsin is investigating last week's suicide of a mental patient at Waukesha (WI) Memorial Hospital, the Milwaukee Journal Sentinel reported.

Wendy L. Snyder, 43, shot herself in the head after returning to the mental health unit with a handgun she took during a visit to her home. Snyder was a mother of four. Her death marked the second suicide at the hospital in under three years.

In 2002, a patient jumped to his death from the top floor of the hospital parking ramp. The state Bureau of Quality Assurance investigated that death and found the hospital deficient in its operation of the unit.

The Bureau of Quality Assurance, which licenses hospitals, is launching an investigation into the operation of the mental health unit, police confirmed.

However, police refused to release reports detailing any more information about the suicide.
