Kentucky Gov. Ernie Fletcher's administration, already strapped for Medicaid cash, fears a mass conversion of physician offices to rural health clinics and primary care centers, according to The Associated Press.

Kentucky officials said physicians in some areas decided to switch to get higher fees from Medicaid, the state-federal health insurance program for the poor and disabled. State Medicaid Commissioner Russ Fendley said getting designated a rural health clinic or primary care center is a fairly simple paperwork matter.

Medicaid pays $27 for a physician office visit but up to $200 for a visit to a clinic or care center. The average is about $90, Fendley told the AP.
