Inconsistency mars first error reporting effort in CT

The first set of adverse event reports from Connecticut hospitals and ambulatory surgical centers provides inconsistent and incomplete information, according to the Hartford Courant on May 30.
The organizations released the reports in October and November after the state passed a law requiring them to track and disclose medical errors. However, reporting inconsistencies in the documents make trends difficult to identify, according to the Courant. For example, one Hartford hospital disclosed 15 errors over a two-month period, while another disclosed only one. Other reports show errors, but don't disclose the ultimate status of the patient.
Of the 94 adverse events contained in the reports, 20 stemmed from errors during laparoscopic surgery, according to the Courant.
