CA power outage causes mass medication labeling errors

A California power outage may have caused thousands of patients to receive incorrect prescriptions on March 13, according to the Associated Press (AP).

Patients who filled their prescriptions at Kaiser Permanente pharmacies either in person or over the phone may have received mislabeled drugs after the outage threw the pharmacies' labeling software out of synch. Consequently, patients may have received an incorrect medication, even if the label was correct.

Representatives from Kaiser Permanente hospitals contacted all but 152 of the patients who may have been affected by the error within four days. Hospital workers visited some patients at home, but were unable to contact those who listed post office boxes as their address, according to hospital spokesperson Lea Rubio.

As of March 17, no patients reported any adverse reactions because of the error, according to the AP.
