By far the most frequent violations in 2008 for offices and clinics of doctors of medicine were:
- Bloodborne pathogens
- Hazard communication
Also issued were violations for:
- Electrical general requirements
- Wiring methods, components, and equipment for general use.
- Portable fire extinguishers
- Exit routes
- Personal protective equipment for eye and face protection
