IAC offers Spanish flu shot declination form

Obtaining documentation from healthcare workers who decline influenza vaccinations is part of the current vaccination recommendations from the CDC and The Joint Commission (formerly JCAHO). To help in that effort, the Immunization Action Coalition (IAC) has developed a Spanish-language version of the declination form.

The IAC grants free use of the form for healthcare employers who recommend that their employees receive annual influenza vaccination. The document provides reasons for vaccinating healthcare workers against influenza and space for the employee's signature if the employee declines the vaccination offer.

Last year the IAC created an English-language version of the form that is still available for use.
