Time's up for the "old" CMS-1500 form

The CMS-1500 (12/90 version) form, which CMS originally allowed to be used past the April 1 cut-off date, has reinstated its decision to terminate the use of the form, according to a CMS press release. Starting July 1, the new form, CMS-1500 (08/05), must be used, and June 29 is the last day the older version will be accepted. Originally, CMS thought a printing problem would prevent software from accurately reading the form. This has not been the case and CMS has since recanted its previous announcement, according to the press release.
