CMS extends deadline for Form CMS-1500 (12-90)

CMS extended the deadline past the April 1, 2007, cutoff for changing from Form CMS-1500 (12-90) to CMS-1500 (08-05), which will accommodate the National Provider Identifier (NPI). This will not affect the May 23 implementation of the NPI, according to a CMS press release. Originally, CMS planned on making the Form CMS-1500 (08-05), which SNFs sometimes use to bill Medicare Part B, the only acceptable form after the April 1 deadline. But print vendors received improperly formatted forms from the National Uniform Claim Committee resulting in the printing of unusable documents, which do not comply with CMS' specifications. The correct form contains, in the upper right-hand corner a one-quarter inche gap between the tip of the red arrow above the vertically stacked word "CARRIER," according to the press release.
