New Medicare providers must use new form to enroll

As of May 1, therapists must use form 855I to enroll in the Medicare program for the first time, according to PT Bulletin Online.
The new form requires providers to submit their National Provider Identifier (NPI) on the application, effectively moving up the NPI application deadline for new providers.
Existing Medicare providers still have until May 2007 to obtain their NPI, unless their contractor asks them to validate their enrollment application or submit changes to their enrollment information.
In addition, Medicare is requiring new providers to submit an electronic funds transfer form (CMS-588).
