Optimizing Reimbursement: Weekly tools and tips

Treating therapists must be accurate in their billing and documentation. Each day after delivering treatment, therapists should check that:
• They adequately documented the services billed
• The services billed for were authorized on the plan of treatment
Daily documentation should prove that procedures and modalities were performed. However, it is not necessary to write a full assessment of the patient and justify the need for skilled services on a daily basis. It is sufficient to write a weekly summary that identifies the skilled interventions provided and the procedures and modalities that facilitated progress toward goals and to make a statement that justifies the continued need for therapy services to address the deficits identified upon evaluation and progress to the identified goals.
