Improve your documentation with specific questions

A patient’s ability to perform activities of daily living (ADLs) is often just a generic statement in most therapists’ evaluations. Improve your documentation by asking patients specific questions regarding what ADL tasks they can perform independently and with which ADLs they require assistance since their injuries or illnesses. Specifically address the patient’s ability to do the following.

  • Cook and prepare meals
  • Complete laundry
  • Complete household cleaning, including vacuuming, dusting, cleaning the bathroom, etc.
  • Make a bed
  • Perform other general household task

This tip is from HCPro’s book The How-To Manual for Rehab Documentation: A Complete Guide to Increasing Reimbursement and Reducing Denials, written by Rick Gawenda, PT.
