Demographics is one of the major components of any financial assistance application. You'll need the patient's name, a list of family members living in the same household, and their address.
Once the patient lists all members of the family, ask for a numeric confirmation of the size of the family. This will help you confirm that no family member has been accidentally missed.
This is extremely important. The size of the family is critical, as the federal poverty guidelines use the size of the family in conjuction with income during the eligibility determination.
