Stop readmissions when you can: CMS plans to eliminate unnecessary return visits

In April, CMS announced the beginning of a pilot project called Care Transitions, aimed at eliminating unnecessary hospital readmissions. With this in mind, it’s time to become more conscious of your facility’s admission and discharge practices.
CMS data show hospitals will readmit one in five patients within a month, CMS acting administrator Charlene Frizzera stated in a press release. More than three-quarters of these readmissions are potentially preventable.
