Rotation scheduling: Eliminate duplicate rotations

Program coordinators should look at the minimum number of cases that residents need in each category, and they should monitor residents’ caseloads to determine where residents complete those index cases and how many cases they accrue. If residents accumulate more than required, the program may choose to eliminate duplicate rotations.
 
For example, residents must have a minimum of 50 vascular surgery procedures. Each PGY level participates in a vascular rotation and averages 100 vascular cases. Use rotation reports and resident evaluations of faculty to determine on which rotation residents receive the best educational experiences in vascular surgery. Alter schedules to eliminate the duplicated or unproductive experiences.
 
This approach has both positives and negatives, which include the following:
  • Positive: Residents maintain the minimum numbers for specific areas and duplicated experiences are removed from the schedule, freeing up time for other educational experiences.
  • Negative: Without the residents, the department affected by the eliminated rotation may need to augment its patient care services. The department may increase attending physician coverage or hire physician assistants and registered nurse fi rst assistants to replace the residents. Both options have associated costs that affect either the attending physician’s practice plan or the hospital.
