The ACGME implemented its Clinical Learning Environment Review (CLER) program as part of the Next Accreditation System to encourage health systems, medical centers, teaching hospitals, and other clinical settings to better engage residents and fellows in six focus areas: care transitions, duty hours and fatigue management and mitigation, patient safety, professionalism, quality improvement, and supervision.
