Utah public health officials are pushing for greater patient safety error surveillance in the state's hospitals. The hope is that greater surveillance and data collection can help identify and eliminate common patient safety problems, according to the Salt Lake City Deseret News.

The state currently uses voluntary reporting for medical errors, but only has part-time staffers to analyze the information gathered. Other options, such as chart reviews and increased use of triggers to find uncommon treatments.
