IHI white paper advises how to handle serious adverse events

The Institute for Healthcare Improvement (IHI) has released a new free white paper on managing serious adverse events.
The white paper, Respectful Management of Serious Clinical Adverse Events, introduces an approach for creating a plan before events occur, as well as how to handle events if they occur before such a plan is implemented. The white paper includes three tools: a checklist, a work plan, and a disclosure culture assessment tool.
