New Jersey Hospital Association launches program to prevent heart failure rehospitalizations

On June 17, the New Jersey Hospital Association (NJHA) launched a collaborative program to reduce the number of patients that are rehospitalized due to heart failure. The year-long initiative will include hospitals, home health and hospice agencies, and long term care facilities. Those involved will be educated by industry leaders and be able to share best practices, data, and strategies for preventing these types of readmissions.
The collaborative will specifically look closer at why patients are being readmitted as well as develop resources for preventing readmissions and managing heart failure patients, and translate the lessons learned into advice and tools for other patient populations.
