HRET releases guide to help hospital leaders prevent readmissions

The Health Research and Educational Trust, an affiliate of the American Hospital Association, released on Monday a guide to help hospital leaders address the prevention of readmissions. The guide outlines the following four steps:
- Examine your hospitals current state of readmissions
- Assess and prioritize your improvement opportunities
- Develop an action plan of strategies to implement
- Monitor your hospital's progress
The guide offers strategies to leaders that have been shown already to reduce preventable readmissions. It also works with the existing national strategies from the IHI and The Joint Commission on the topic.
