Q. How should we notify patients and families that they can activate the rapid response team? What wording could we use?

A. There are several important elements associated with National Patient Safety Goal (NPSG) 16.01.01, which says facilities must choose a suitable method that enables staff members to directly request additional assistance from specially trained staff members when a patient’s condition worsens.

Element of performance (EP) 2 requires the hospital to develop criteria by which staff members should judge whether it is necessary to call additional assistance to respond to a change in a patient’s condition or whether there is a perception of a change by staff members, the patient, or the patient’s family.
