In Scotland, medication errors affect five patients per day

Five hospital patients per day are put at risk of becoming a medication error victim according to an investigation by Scotland on Sunday.

Errors included one situation in which a baby was given vaccines before the recommended minimum age and another in which a pregnant woman was given drugs that may cause birth defects, reports Scotland. The data also includes "near misses" in which errors are caught before reaching the patient.
