Reason for visit sticking point when billing injections

More often than not, injections are bundled into the evaluation and management (E/M) level and cannot be billed separately for Medicare and carriers who follow Medicare guidance. However, there are a number of cases where it's appropriate to code and bill for injections separately. It all boils down to the reason for the visit.
