Emphasize results in your documentation
HCPro's Weekly Update on the ANCC Magnet Recognition Program®*, August 22, 2006
If a particular program or initiative does not have quantifiable outcomes, then do not include the evidence in your documentation. For example, you may have what you believe to be an innovative multifaceted program to prevent or reduce the incidence of ventilator-associated pneumonia. However, if you cannot provide evidence of sustained prevention and control of ventilator associated pneumonia, the example should not be included in the documentation. Most of the exemplars that your organization chooses to feature in the documentation should describe how your nursing organization was able to recognize problems, establish plans to address the problems, apply the plans, measure changes attributable to the plans, and revise those plans as necessary.
