Seven questions to help you choose your written documentation theme(s)

One strategy for developing the theme for your written documentation is to look at your mission, vision, and values, and ask yourself the following questions:
- What are we known for in the community?
- What special awards has our organization received?
- Are there specific positive themes that have emerged from our patient satisfaction survey results?
- Why do doctors bring their patients to our hospital?
- What do our patients say about our care?
- Why do our nurses like working at the organization?
- What makes nurses stay?
Usually, a thorough examination of the organization's values helps to set one or more themes for your written documentation. Once the theme(s) are established, you will have an easier time selecting the evidence for your written documentation.
