'Keeping it real' in the documentation

If every example in your written narratives seems too good to be true, or if every attempt at change ends exactly as planned, the appraisers may wonder whether they're getting the entire story. Add some of the more difficult struggles to your success stories to illustrate your organization's perseverance and fortitude over time, suggests Barbara A. Wadsworth, RN, MSN, MBA, CNAA,BC, interim vice president, patient services, at Abington (PA) Memorial Hospital. Focus on actual events, meaningful dialogue, and work efforts that supported staff at all levels of the organization through challenging circumstances (e.g., high vacancy rates). That is really what matters in today's challenging and evolving healthcare environment. What you do in the face of a tremendous challenge speaks far more about your organizational character than what you do when things are going well.
